You just include all the necessary information needed (Add'l contact phone #'s, Medications, Allergies, Important Medical History, Dr's Name and phone number plus any Insurance policy information etc) & we will put it on your card & laminate it so it will be safe from moisture and tearing.		 This the front of the card.
